Eligibility Forms for Drugs Funded by the NDFP
Eligibility forms specify for physicians and pharmacists the reimbursement criteria for intravenous cancer drugs, drug combinations and indications currently covered under the New Drug Funding Program (NDFP).
The NDFP provides reimbursement to more than 80 Cancer Care Ontario partner hospitals for intravenous cancer drugs that have been approved for coverage and are administered on site. Private clinics, individuals and retail pharmacies do not qualify for reimbursement through this program.
Eligibility forms must be completely filled out and submitted by the prescribing physician before treatment begins. Completed eligibility forms and supportive documentation, if applicable, must be submitted to Cancer Care Ontario according to the published submission deadlines and policies.
